The Critical Care Transitions Team

“Your fight is our fight, and our results prove that we can make incredible things happen”

We work closely with case managers and pediatricians in NICUs and PICUs throughout the state, and their number one concern is always the same. They worry about their patient’s transition from hospital to home, and they often feel that families and home healthcare agencies aren’t fully prepared for the complex process.
We have listened to these valid concerns and have designed a solution: a critical care transition team. Our specialist team brings together the most qualified and experienced professionals in pediatric home healthcare to ensure that your child is kept safe during their transition. When you choose to work with Family First Homecare we will assign your family with a highly skilled team including a pediatric clinical care liaison, a respiratory therapy care education director, a critical care RN case manager, and a pediatric operations manager. This team will work with you, your child’s physicians and their NICU or PICU case manager to develop an in-depth care plan to get your child home safely.

We are Committed to Quality Care

  • Our skilled nurses have the best interests of their patients in mind, and they consistently deliver the highest quality of care. Our warm and thoughtful team will always be completely transparent, so you’ll know exactly what’s happening in your child’s treatment every step of the way.
  • Our communication with doctors and specialists continues after your child has transitioned from hospital to home. We keep in close contact with specialists and physicians to ensure the best possible clinical outcomes for your family.
  • Our training program is second to none, and our highly qualified team is committed to delivering the highest standard of care. Each nurse on our team is provided with specialist training relevant to their patients and the equipment and techniques that they use.

Our Focus is on Education

Our dedicated nurses undertake comprehensive respiratory training which is delivered by senior specialists. Learned skills including:

• Adult and pediatric respiratory care
• Durable medical equipment (DME) used with respiratory homecare patients
• Respiratory-related diagnosis common in homecare
• Tracheostomy and ventilator management

Providing the Highest Standard of Pediatric Homecare

Around 2% of children born in the United States are born with a disability severe enough to need life-long care and special services. Those first few months in the NICU are incredibly tough for families, and the fight continues long after you transitions back home. The journey may, at times, seem impossible. But we’ve witnessed firsthand the incredible strength and resilience of families who are fighting for their child.

We are with you every step of the way. Your fight is our fight, and we’ll be at your side during every setback and every victory. Family First is made up of mothers, fathers, and caregivers who have experienced this journey either personally or professionally. Your family and your outcome are at the heart of our service, and we are deeply committed to helping your child thrive.

Family First was built on the belief that anything is possible, and we are dedicated to redefining the standards of pediatric homecare so more and more families can achieve the outcome they hope for.

It is our committed approach that leads pediatricians, NICU and PICU nurses, case managers and specialists throughout Florida to request our services for their patients. We have earned our reputation as one of the leading pediatric homecare services by continuously working to raise the bar.

From the very first phone call with our office and throughout our care plan, we are there for you. Our team is given the very best training and our nurses treat your family as if it was their own. Together, we can make great things happen.

Family First Homecare has always been very responsive in relaying time-based information. The process of clear and consistent communication when excepting referrals has reduced the frustrations that occur with hospital to home transitions. Family First has excellent customer service as they communicate with well-planned information when accepting one of our critical care children, as well as putting together a clear concise action plan.
NICU Case Manager
Family first Homecare is the best agency that I’ve ever worked with and I’ve worked with quite a few in the care of my daughter. Not only are they high quality of service and excellent communicators, they are very competent compassionate people. Their nurses go through advanced training for tracheostomies and ventilators which is provided directly by Family First. I have nothing but praise for their Director of Nursing and Administrator and for the interventions that they have done on my behalf in caring for my child. They treat my child as if she were their own.
Parent of Critical Care Family First Homecare Patient
Family First is very reliable and is always willing to look at referrals to see if they can provide services. They are willing to meet with the family and try to work it out instead of just declining the referral. All of our families that have Family First are very happy with the services being received.
