Getting you home from the hospital shouldn’t be stressful.

We work closely with case managers and pediatricians in NICUs and PICUs throughout the state, and their number one concern is always the same. They worry about their patient’s transition from hospital to home, and they often feel that families and homecare agencies aren’t fully prepared for the complex process.

We have listened to these valid concerns and have designed a solution: our Critical Care Transitions Program. Our specialist team brings together the most qualified and experienced professionals in pediatric homecare to ensure that your child is kept safe during their transition. 

When you choose to work with Family First Homecare we will assign your family with a highly skilled team. This team will work with you, your child’s physicians and their NICU or PICU case manager to develop an in-depth care plan to get your child home safely.

As a parent, if you are new to the homecare field or feeling overwhelmed about your child being ready to leave the hospital to come home for the first time, we understand. We have developed our program for this very reason, whether the transition needs to occur from one agency to Family First, or from the hospital to home.

Care Coordination

Coordinating multiple care providers can be overwhelming, which is why we offer a comprehensive service to coordinate the provision of care to your child. We develop strong and collaborative relationships with the medical communities who share the responsibility of identifying, prescribing and delivering the right care.

Hospital Rapport

The rapport we keep with doctors and specialists doesn’t end when your child leaves the hospital. We maintain consistent communication with your physicians and provide the visibility they need to assess and ensure your child is receiving care and support they need as developments occur.

Nurse Education

In addition to our team undergoing training specific to the families they are working with, our dedicated nurses are offered comprehensive training including:

  • Tracheostomy and ventilator management
  • Respiratory-related diagnosis common in homecare
  • Adult and pediatric respiratory care
  • Durable medical equipment (DME) 

Pediatric Teaching

We are committed to becoming a pediatric teaching organization as a strategy to improve pediatric nursing across the industry for nurses and therapists who work for any organization. Families are in need of advanced skill sets to safely provide complex care and Family First Homecare is dedicated to delivering it.

Precepting is vital to promoting the competence, confidence and safety of new nurses in a new environment. Which is why Family First created our own Preceptor Program –  an innovative model of preceptorship/mentorship that allows new nurses to learn from and shadow experts in the field, helping to meet the unique needs of homecare nurses to be successful with their patients and provide the highest quality care. This ensures we continue to push homecare to its highest level of quality for our patients possible.
